Nasrin is an Adult Gerontology Primary Care Nurse Practitioner. She attended University of Colorado for both her BSN and MSN degrees. She is passionate about caring for the older adult population. She finds that home-based primary care enhances patient-centered care and relieves the burden and stress for patients when arranging transportation to appointments. Bloom’s model of care allows care planning to prioritize the patient’s individualized goals. This model empowers patients to remain active in their care without needing to leave their home. In her free time, she enjoys time with family, spending time in nature and doing arts and crafts.
