What is the GUIDE Model?
The GUIDE Model is a CMS Innovation program designed to improve care for people living with dementia and support their family caregivers. The model provides funding for coordinated, person-centered dementia care through an interdisciplinary team—led by a primary care practice.
Who is eligible for GUIDE Services?
Patient must have traditional Medicare (not Medicare Advantage)
Patient must have a dementia diagnosis of any stage or type
Patient must not be on Hospice or in a long-term care facility (Memory Care and Assisted Living are eligible)
