Corin Reyes, MSN, RN, FNP-C, CHW

Nurse Practitioner

Corin earned her Master of Science in Nursing (FNP) from South University, her Bachelor of Science in Nursing from Schreiner University, and her Associate of Science in Nursing from Lamar State College, building on 17 years of nursing experience that began as an LVN. She is a Certified Community Health Worker through the Texas Department of State Health Services and holds additional certifications in Social Determinants of Health (University of Michigan), Diversity, Equity, and Inclusion (University of South Florida), Trauma-Informed Care, Mental Health First Aid, and Organizational Health Literacy.

During the pandemic, Corin partnered with Meals on Wheels, San Antonio Metro Health, and a sorority sister to build a program delivering in-home vaccines to homebound seniors — and what began as an immunization effort quickly opened her eyes to everything traditional healthcare was missing: the food insecurity, the fall risks, the isolation, the unmet needs that never make it into a clinic visit. That experience reshaped her understanding of what comprehensive care really looks like and why the home is often the most honest place to practice medicine. Bloom’s model resonates with her because it meets people where they are, restoring the dignity, continuity, and full-picture care that modern healthcare too often loses.

Corin is a mother of two, a longtime dancer, and the founder of a nonprofit supporting women. She is bilingual in English and Spanish, still lives on the same San Antonio block where she grew up, and believes deeply that healing should feel joyful, embodied, and personal.
