Bloom Healthcare is Excited to announce

we have partnered with Medicare on a new initiative where health care providers who share common goals aimed at improving patient care can work together more effectively. This initiative allows health care professionals, like Bloom Healthcare, to form what is called a “Direct Contracting Entity” or “DCE”, which allows them to work together with the Medicare program and other healthcare professionals to give you more coordinated care and better service.

Bloom Healthcare and our primary care providers were chosen to participate in this new initiative, and we are excited to announce we have formed a DCE also called Bloom Healthcare, because we think it will help us provide better quality care for you and the rest of our Medicare patients.

Bloom Healthcare believes that you would potentially benefit from the enhanced benefits and services offered by this new initiative.

Same Insurance

You do not have to change your insurance. You will keep your Medicare coverage and your healthcare provider of choice! This Medicare program will help us better identify the care and services you need, reduce paperwork and administrative hassles in order to provide better care, improve health outcomes, and lower the cost of care!

CMS - Direct Contracting Program

  • Simply use the form below to confirm that Bloom Healthcare is your main doctor or health care professional for routine primary care, to help determine if Bloom Healthcare can help provide better care and benefits for you. You will receive routine primary care in your home or place of residence from our primary care providers. Routine care can include regular care and check-ups you get from a doctor or other health care professional and care for other chronic health problems, such as asthma, diabetes, hypertension, CHF, COPD, etc.

  • Note: Whether or not you complete this form or select Bloom Healthcare as your main doctor or other health care professional, you remain eligible to receive the same Medicare benefits and you still have the right to use any doctor, other health care professional, or hospital that accepts Medicare, at any time. This form will simply help determine if Bloom Healthcare can help provide improved care and better service for you.

  • Selecting Bloom Healthcare or any other doctor or other health care professional by completing this form is your choice AND you can change your mind later. If you complete this form and select Bloom Healthcare (or any other doctor) as your main doctor or health care professional, you should do so yourself. No one else, other than a Power of Attorney or Legal Guardian, should complete this form for you.

If you make a selection on this form and make a different selection through, Medicare will prioritize the selection you make through No one is allowed to attempt to influence your choice to complete this form or select a doctor or other health care professional through by offering or withholding anything in exchange for you to complete or not complete the form or to make a selection online.
